USPS Form 1583, explained (2026)
By Same Day Virtual 路 Updated
The short answer
PS Form 1583, Application for Delivery of Mail Through Agent, is the Postal Service form that lets a commercial mail receiving agency (CMRA), such as a virtual office or a mailbox service, receive mail for you. You list who will get mail there, provide two IDs (one government-issued photo ID and one that confirms your address), and sign in front of the agent, with the agent on a live video call, or before a notary. The agent uploads the form to the Postal Service and keeps a signed copy.
What the form is for
The current edition is PS Form 1583, June 2024, titled Application for Delivery of Mail Through Agent. The "agent" is the business that receives mail on your behalf. A virtual office address is one of those businesses, and so is a mailbox at a mailbox store.
According to the form, the agent uploads the original completed and signed form to the Postal Service's CMRA Customer Registration Database and keeps the signed copy at its business location, where it must be available at all times for examination by the postmaster and the Postal Inspection Service. The Postal Service may also run its own verification on the application.
The two IDs it asks for
Two types of identification are required for the applicant and for each authorized individual listed on the form. One must be a government-issued photo ID. The second must confirm the address you list on the form. A copy of both is attached to the form.
| Photo ID (one of these) | Address ID (one of these) |
|---|---|
| U.S. state, territory or tribal driver's license or non-driver ID card | U.S. state, territory or tribal driver's license or non-driver ID card |
| Uniformed Service ID | Current lease |
| Passport | Home or vehicle insurance policy |
| Certificate of Naturalization | Mortgage or deed of trust |
| U.S. Access Card | Vehicle registration card |
| Matricula Consular | Voter card |
| U.S. Permanent Resident Card | |
| U.S. university ID card | |
| NEXUS card |
A driver's license or state ID can count as the photo ID or as the address ID, but not as both. Bring a second document from the other column.
How the signature is verified
The form gives three ways to sign. You sign, or confirm your signature:
- in person, in front of the agent or the agent's authorized employee;
- in the agent's virtual presence, over real-time audio and video; or
- by acknowledging your signature before a notary public commissioned in a U.S. state, territory, possession or the District of Columbia, in person or over real-time audio and video.
A signature on its own, with nobody verifying it, is not one of the options.
Completing Form 1583 with Same Day Virtual
- Choose an address on the locations page and check out online.
- In your client portal, upload the completed and signed Form 1583 and your two IDs. The portal shows which of the three documents are still missing.
- Sign the form in one of the ways listed above. If you are not sure which one to use, call (213) 232-3820 before you sign.
- Under our Terms of Use, the form has to be completed within 30 days of starting service.
Anyone else who should receive mail at your address, such as a partner or an employee, needs to be covered by their own Form 1583 before mail can be accepted for them. Adding recipients to an account can change the monthly fee; the Terms of Use explain how.
When you stop using the address
The form's agreement section sets out three points for when the agency relationship ends:
- neither you nor the agent may file a change-of-address order with the Postal Service for mail delivered to the agent;
- moving your mail to another address is the responsibility of you and the agent; and
- mail the agent puts back into the mail for you has to be paid again with new postage.
So before you leave any mail service, update your address with your bank, customers and the agencies you filed it with. Our Terms of Use also say that held mail may be kept for six months or longer and that fees for handling, forwarding, postage and scanning still apply while it is held.
Questions
Can I complete USPS Form 1583 online?
The signature is the part that needs care: it has to be verified. The June 2024 form allows signing in the agent's virtual presence over real-time audio and video, or acknowledging the signature before a notary public, which can also be done over real-time audio and video.

Can I use my driver's license for both IDs?
No. The form says a driver's license or state ID card may be used for only one of the two IDs, either the photo ID or the address ID. The second ID has to be a different document, such as a passport for the photo ID or a current lease, insurance policy, vehicle registration or voter card for the address ID.

Does everyone at my company need to fill out Form 1583?
Everyone who will receive mail at the address has to be covered. The form asks for two IDs from the applicant and from each authorized individual listed. At Same Day Virtual, a person who is not on your form needs their own Form 1583 before mail can be accepted for them.
How long do I have to submit Form 1583?
With Same Day Virtual, the Terms of Use require the form within 30 days of starting service. Other providers set their own deadlines.
What happens to my mail when I cancel?
The form says neither you nor the agent may file a change-of-address order with the Postal Service, that moving your mail is the responsibility of you and the agent, and that mail sent on again needs new postage. Update your address everywhere you used it before you cancel.
